After a complete history and exam, Dr. Michael sits down with you and explains your problem from a chiropractic reference. She will confirm whether or not you are a candidate for chiropractic care. Then she will review your treatment plan, how long your acute phase will take, and the cost of care. Dr. Michael balances your nervous system and helps you learn to take better care of yourself. You may be required to put on a treatment gown to access your areas of complaint for your adjustment, physical therapy, or massage. If your problem is a lower extremity, please bring a pair of shorts.
Insurance;Upon request, we will provide you with a receipt with the correct codes for you to submit to your insurance for reimbursement.
By not getting involved in billing your insurance directly, Dr. Michael is able to keep her fees reasonable. It is her intention to focus on the quality of your care and provide you with her best possible treatment.
MEDICARE Patients;It is our regret that Medicare will not properly reimburse our patients. We do not take MEDICARE assignments currently.
Payment;Checks, Credit Cards, and Cash only. We are a cash-based practice. Payment expected at the time of service.
Credit charges will have a service charge of $3.00.
Upon request, we supply a receipt you can submit to your insurance company.

New Patient Includes: complete history, exam, and treatment - $130
(Does Not Include: supplements, x-ray, MRI or other special studies)
The initial visit is one hour but if you need to fill out the forms in the office, time in the office then will generally be 1.5 hours.
Structural Balancing Includes: exam and spinal adjusting - $70

Every patient must fill out and sign three forms before the start of the first appointment. If you did not bring them or have not arrived at least half an hour before the scheduled appointment to fill them out you will likely need to reschedule your appointment for another time.
The forms can be downloaded from the links provided in the below. If you do not have access to a printer or would rather fill them out at the office, please arrive 1/2 hour before your scheduled appointment. Wear or bring loose clothing. If you have an extremity problem (ie, knees) please bring or wear shorts.
